The Apparatus Passport is 5.5" x 3.5" UV stable plastic with your Apparatus ID engraved at the top and your department name engraved on bottom. It works with a Velcro system featuring hook on the back to attach to loop on the apparatus (included) and loop on front to attach personnel name tags (sold separately).
